Subject Access Request

Summary

A Subject Access Request (SAR) is a request to an organisation (under the Data Protection Act 2018) for a copy of personal data held by that organisation.

For personal requests, you must provide two forms of identification; One to confirm your identity (Driving licence, Passport, Birth Certificate, Marriage or Civil Partnership Certificate or NHS Digital Identity Badge), and one to confirm your current address (Utility bill, Bank Statement, Credit Card Statement, Benefit Book or Pension Book).

For Solicitor requests, you must provide a consent form dated within 12 months of requesting.

A request on behalf of someone (living or deceased) you must provide your identification and one of the following:


Request on behalf of:Document required
Child or young personCopy of the child's full birth certificate
Deceased patient

- Copy of the Will stating that you are the executor of the deceased's estate;

- Probate form stating that you are the deceased's personal representative

- Claim arising from the patient’s death & wish to access information relevant to your claim

In the event that one of these cannot be provided your request will be forwarded to the Information Governance team to be approved.

Living patient

Signed letter of consent from the patient that confirms authorisation to act on their behalf; link to hosted pdf

or

Court order stating that you have the power of attorney for Health and Welfare




How to make a request

How do I make a request?

When making your request, please ensure the correct application type is selected.  This will enable the team to process your request effectively.  

You can get a copy of your health records by creating an account through our online portal: Account Signup.

If you are an organisation or representative, you should also use the online portal: Account Signup.

If you are applying to access the records of more than one person you must complete a separate application for each patient.

This portal is to be used to request Medical Records and Imaging from the University Hospitals of Leicester and the Alliance Community Hospitals

Community Hospitals

If you require any Alliance Community records for the hospitals listed below,

  • Coalville Community Hospital
  • Feilding Palmer Hospital
  • Hinckley and District Hospital
  • Melton Mowbray Hospital
  • Rutland Hospital
  • St Lukes Hospital
  • Loughborough Hospital

The Alliance provides records for Outpatients, Day Case Surgery, Radiology and Endoscopy. All other records for the above hospitals would be provided by Leicestershire Partnership NHS Trust (LPT), please contact lpt.sarrequests@nhs.net
